If you do not have health insurance or choose not to use your insurance benefits for services at Horizon Counseling Center, you have the right to receive a Good Faith Estimate of the expected cost of your care before services are provided.A Good Faith Estimate outlines the anticipated charges for services based on your expected treatment plan. Because treatment needs may change over time, the estimate may be updated as appropriate.
Please note: This Good Faith Estimate requirement applies only to uninsured or self-pay clients. If you are using your health insurance benefits, your out-of-pocket costs are determined by your insurance plan and may include deductibles, copayments, coinsurance, or other cost-sharing amounts. Horizon Counseling Center cannot guarantee your insurance coverage or final financial responsibility. We will gladly verify your benefits as a courtesy and explain the information provided by your insurance company prior to your first appointment.
If you receive a bill that is $400 or more above your Good Faith Estimate, you may have the right to dispute the charges through the federal patient-provider dispute resolution process.
